Dental radiology makes use of specific types of equipments needed for imaging purposes in dentistry. Frequent exposures though with low doses involve a risk for the practitioner and for the patient. Risks may vary from the increased occurrence of cancer, birth defects, cataracts and even shortening of life span. Hence it is mandatory for a dental practitioner to be familiar with the magnitude of radiation exposure, the possible risks and the methods used to keep the effective doses resulting from the imaging procedures as low as reasonably achievable.

Dentists perform a variety of surgical procedures in their daily practice which frequently require the use of haemostatic agents. The purpose of this presentation is to provide an overview of the causes of perioperative hemorrhage and the use of haemostatic agents. The current concepts in the treatment of patients on anticoagulant therapy, antiplatelet therapy and those with congenital coagulation defects will also be discussed.

The presentation will discuss the challenges when dealing with pediatric dental patients, which include the behavior management, the nature of dental caries in children with emphasis on early childhood caries. The different modalities of dealing dental caries in children, the latest recommendations for caries control in primary teeth & pulp therapy including acceptable diagnostic criteria, will be discussed. The advanced methods for pharmacologic behavior management including nitrous oxide anxiolysis, conscious sedation and treatment under general anesthesia will also be discussed.

In contemporary clinical orthodontic practice, great emphasis is given to non extraction strategies .This is mainly because non extraction cases result in better esthetic soft tissue facial profile, smile characteristics, functional occlusion without much of the unaesthetic buccal corridors. To achieve these objectives orthodontists take all efforts to use the various strategies at their disposal. Maxillary expansion is one such strategy. The presentation will discuss various methods and the appliances available towards this, illustrated with clinical cases.

Dental lasers have been a controversial topic from the time Maiman introduced them in 1960 and have undergone many experiments and studies. With the newer advances in research and technology in the field of laser dentistry, we can now make our daily dental practice much easier and faster with laser assistance. Currently numerous laser sources are available, each having its own special properties. The understanding of these properties is a precondition for using lasers appropriately. Dental lasers are widely used in practise for better and long term good results. They can be easily integrated for routine dental procedures with minimal side effects provided they are used according to the proper protocol and power settings. The most important aspect in integrating lasers in a dental practice is the understanding of the biophysical interaction between the different laser wavelengths and different tissues.

The goals of periodontal therapy are arresting the disease process, preventing disease recurrence and regenerating periodontium, lost as a result of periodontal disease. Wound healing is a complex, well-orchestrated sequence of events. In studying the dynamics of cell-to-cell and cell-to-tissue interaction, scientists discovered the presence of growth factors. The presentation will discuss the techniques currently in use, which include open flap debridement, use of bone grafts, bone substitutes, guided tissue regeneration, combination techniques, root surface treatment and the use of biologics. It is also documented that these procedures in clinical periodontics and experimental studies have predictable results.

Ever since the introduction of rotary nickel titanium (RNT) instrumentation, choosing between the existing RNT systems has, largely, been a decision between safety and efficiency. The endodontic Glidepath is the secret to radicular rotary safety. This presentation will discuss the definition of Glidepath, the importance in producing optimum endodontic mechanics, and describe the preparation of a Glidepath for radicular shaping. The obstacles to Glidepath preparation and the solutions to overcome them will also be discussed to produce a safe rotary result and a good endodontic experience.

Abstracts of CME Talks on Renal Disease in General Practice

Talk 1: Chronic Kidney Disease: A Global Burden.

Speaker: Dr.Abraham George, Consultant Nephrologist, Royal Hospital, Sharjah

Chronic Kidney Disease (CKD) has become a global financial burden and in UK it consumes 2% of the NHS budget. The National Service Framework for Chronic Kidney Disease that was published in 2004 by the NHS, UK planned a 10 year vision to set standards and guidelines for these patients.

In this lecture we will be discussing the basics of the kidney function very briefly, how to interpret the creatinine values and thereafter will go on to the various issues or factors related to the chronic kidney disease including Obesity, Diabetes Mellitus, Hypertension and more. It is necessary to understand the staging of CKD because of its practical implications and highlight the importance of proteinuria and CKD in cardiovascular mortality. Current management principles and referral procedures will also be mentioned.

Talk 2: Renal Disease in Pregnancy.

Clinicians encounter many medical problems during pregnancy such as hypertension, proteinuria, pre-eclamptic toxemia (PET) and renal failure. A lot of questions come to your mind like; what medicines are safe in pregnancy for hypertension, when should pregnancy not be advised, when would you consider a therapeutic termination of pregnancy (TOP)?

The lecture will cover a broad range of topics including the physiological changes that occur in the human kidney during normal pregnancy and current principles and management of renal disease including pregnancy in the renal transplant recipient. Over the years, pregnancies have become very successful in patients after kidney transplant with over 95% success rates. Dialysis can reduce the fertility rates, where as in transplant, fertility is normal. We will also be discussing when to refer, when to admit the patients and the specialist management of HELLP syndrome.

Talk 3: Approach to Proteinuria & Hematuria.

Hematuria and Proteinuria are common conditions we find in our practice. The most common causes of hematuria are urinary tract infection, kidney stones or ureter stones, benign prostatic hyperplasia, in older patients. Basically there are two types of hematuria; glomerular and extra glomerular and it is vital to distinguish between them.

Proteinuria means the presence of an excess of serum proteins in the urine which often causes the urine to become foamy and three types of proteinuria are present, glomerular, tubular and overflow. You should remember to rule out transient and orthostatic proteinuria.

Talk 4: Recent Trends in the Management of Renal Tumors.

Speaker: Dr. Waleed Hassen, Consultant Urologist, Johns Hopkins& Al Tawam Hospital, Al ain

This lecture deals with renal masses and the two most common types are renal cell carcinoma and urothelial cell carcinoma. Renal cell carcinoma (RCC), also known as hypernephroma, first described in 1883, is a kidney cancer that originates in the lining of the proximal convoluted tubule. RCC is the most common type of kidney cancer in adults, responsible for approximately 80% of cases. It is known to be the most lethal of all the genitourinary tumors.

Minimally invasive approaches to the treatment of renal malignancies have dramatically evolved and the current management guidelines will be reviewed here. In Europe, kidney cancer accounts for nearly 3% of all cancer cases. In the UK, kidney cancer is the eighth most common cancer in men and in women it is the fourteenth most common cancer.

Talk 5: Acute Kidney Injury: An Update.

Acute renal failure (ARF) or acute kidney injury (AKI), as it is now referred to in the literature, is defined as an abrupt or rapid decline in renal filtration function and it goes way back in the history of medical science to around 2nd century AD.

This condition is usually marked by a rise in serum creatinine concentration or azotemia (a rise in blood urea nitrogen [BUN] concentration). However, immediately after a kidney injury, BUN or creatinine levels may be normal and the only sign of a kidney injury may be decreased urine production. A rise in the creatinine level can result from medications (e.g. cimetidine, trimethoprim) that inhibit the kidney’s tubular secretion. A rise in the BUN level can occur without renal injury, resulting instead from such sources as GI or mucosal bleeding, steroid use, or protein loading, so a careful inventory must be taken before determining if a kidney injury is present.

Talk 6: UTIs in General Practice.

Urinary tract infections are very common in the community affecting entire spectrum of ages in both sexes and the most common type is acute cystitis. Frequent occurrences lead to varied treatment plans by physicians. Inappropriate use of easily available antibiotics, bacterial resistance and special situations require critical evaluation and management.

Current trends in urinary tract infections, diagnosis and management in children, women and geriatric population have to be addressed and guidelines need to be formulated to tackle this common disease effectively. This presentation encompasses these issues and measures for prevention of this illness and effective antimicrobial prescription.

Talk 7: Drug Dosing in Renal Disease.

Speaker: Dr.Sathvik B.Sridhar, Asst.Professor, Dept.of Pharmacy Practice, RAK College of Pharmaceutical Sciences, RAK MHSU, RAK .

Drug dosing is often difficult in patients who do not fit the profile of an average size, average age and otherwise healthy patient and those with decreased renal function are an issue. Inappropriate dosing of drugs can lead to toxicity or ineffective therapy. Decreased renal function can cause decreased clearance of the drug, drug’s metabolites, decreased protein binding of drugs, and decreased ability of the drug to get to the site of action.

The decision of dosage adjustment is often based on four factors such as the therapeutic index of the drug; proportion of renal clearance to total clearance of the drug; renal clearance of active/toxic metabolites and the extent of renal insufficiency of the patient.

Dosage adjustment may involve measures such as extension of the dosing interval, reduction of the maintenance dose and administration of a loading dose or monitoring serum drug levels. Drug dosing errors are common and can cause adverse effects and poor therapeutic outcomes. Dosages of drugs cleared renally should be adjusted according to creatinine clearance or glomerular filtration rate.

Talk 8: Current Concepts in the Prevention & Management of Urolithiasis.

Speaker: Dr. Nazeer Ahmed, Specialist Urologist, Indian Specialist Medical Centre & Aster Hospital, Dubai.

Urolithiasis is a common problem affecting Middle East and is a disease of affluence with high prevalence that has a large impact on the quality of life of those affected. The presentation deals with how to approach a patient with urolithiasis, to understand the basic pathophysiology and the multifactorial etiology of stone formation. The unique phenotypic variation of each individual stone former and the importance of Randall’s plaque as a precursor lesion of stone are stressed.

The preventive measures to reduce the incidence of kidney stone recurrence are explained. The indications for interventional treatment and the varying modes of minimally invasive procedures are briefly explained with video. The evolution of surgical technique appears to have overshadowed the importance of prevention of stone disease despite evidence showing medical therapies significantly decreasing stone recurrence rates.

All AKMG members are reminded to renew their annual membership before March 31st. The Central Working Committee has proposed the following:

Members who have not renewed for two consecutive years should be removed from the membership list. If they have to become members again, they have to pay admission fee again. Please contact your chapter secretary if you have not renewed your annual dues.

The following are the main decisions taken during the AKMG SSS Annual General Body meeting held in January, 2009:

Fines for late payment of annual fee waived till March 31st, 2009. After that the member will have to pay Dhs 25 per month as late fee. After three months of non payment, the member will lose his membership.
Instead of paying the Dhs 200 annual fee for 20 years(Total 4000), SSS members can now take life membership of AKMG SSS by paying Dhs 2000.
Members can continue to be members of the AKMG SSS even if they leave UAE, provided they become life members of both AKMG Emirates and SSS and pay the fraternity charges every year.
Secretaries should collect the SSS annual renewal fee dhs 200 from members preferably as cash, give a temporary receipt, and then send the total amount as a single cheque in favor of AKMG EMIRATES to Dr.Sirajudeen.

For the new members, here are the salient features of AKMG Social Security Scheme:

Retirement benefit when you leave UAE after 3 years of membership
Death benefit to family, in the unfortunate event: To be paid within one month.
Diya money advance if patient under your care dies.

Yesterday we had a family events day committee meeting at Grand Buffet Restaurant, Sharjah.) No, really we were not interested in eating, the Dubai people wanted someplace where they can arrive earlier without much traffic problems.

The following attended: Dr.Viswanathan (President), Dr.Hanish Babu (Secretary General), Dr.Jeladharan (Treasurer), Dr.Sirajudeen (VP incharge), Dr.Sunny Kurian (General Coordinator), Dr.Meera Gopi Kurian (Family activities coordinator), Dr.Feroz Gafoor ( Sports coordinator), Dr.George Joseph (President, Sharjah Chapter), Dr.George Jacob (Ras al Khaima Chapter President), Dr.Chithra Shamsudheen (Sharjah Chapter Secretary), Dr.K.M.Mathew (RAK Treasurer) and Dr. Nisham Noorudeen (Jt.Secretary, AKMG Emirates).

Details of the family day events were discussed, as has been circulated to all concerned. Chapters were entrusted to find sponsors for the event. Registration details, flyer and registration form to be forwarded to all members.

Time: 2.30 pm till 11 pm

Overall in charge: Dr.P.M.Sirajudeen, VP in charge Sports activities, AKMG Emirates
Presidents and secretaries of all chapters are members of the organizing committee.

Sports Activities
1. Volleyball: Knockout games. 4 teams: Abudhabi & Al ain, Dubai, Sharjah, Northern Emirates (Ajman UAQ, RAK,Fujairah). Games start at 3 pm. Teams arriving late: Other team gets walkover.
2. Throwball for Women: Same teams as above
3. Shotput: For Gents and Ladies; individual
4. Three leg race for couple
5. Tug of war: Chapter wise
6. Games for Children
Sports Committee: Coordinator: Dr.Feroz M.Gafoor (Dubai); Members: Dr.Anwar Habeeb, Dr.Viswambaran, Dr.Subhadra, Chapter representatives .

Cultural Programs
Total duration: 2.5 hours
20 Minutes each for a chapter: 5 minutes dance, 5 minutes song, 10 minutes skit ( Need not be same, you can present any program during this time).
The program will be mixed bag, participants will be called randomly. Too much elaborate makeup and properties not needed, so that the participants can enjoy the programs themselves.
